Use the links below to fill out and submit your Initial Health Screening form and your Before photos.

-Please have this form completed by Monday, April 1,

Submit your  BEFORE photos by clicking here. There is also a guide on how to take your before photos.

-Please have your before photos turned in by Monday, April 1.
-If you take PC's with Kristy, we can complete them during your PC.

Measurements are due Monday, April 1. There are a couple ways to get this complete:

-If taking PC's with Kristy, you can do during your PC.
-Measurements can be completed before class on Monday at 6:30pm or after class at 9pm. It will only take about a minute to complete. If you are unable to make any of those times, please contact Kristy to schedule a time.
